can find a GCM in the area where your mother lives. GCM can
act as advocates and liaison for your mother. They will
provide you with written or verbal updates of your mothers
well being. Care Managers will keep you informed of unexpected
events or emergencies. Family members will be involved in any
decision-making. |

I am looking to place my father
in a facility near my home. How do I know what to look for
when looking for a facility? |

Your GCM is knowledgeable about local facilities for senior
care, and will work with you to look at all your options and
assist in making the best recommendations for your father. |

from GCM services, but I do not know if I can afford it. What
does a GCM charge? |

Professional fees vary, but average rates range from $50 to
$150 per hour. Services can be short-term or on-going.
Individual Care Managers will explain their fee structures to
you. |

My dad has Medicare insurance.
Are your services covered by Medicare or other insurance? |

A. |
Most health insurance plans, including Medicare do not cover
the cost of care management services at this time. Some
long-term care insurance plans will reimburse for care
management services. |
