New Jersey Geriatric Care Managers

















Geriatric Care Management services are offered in a variety of settings.

We believe the care management model utilized by our members best serves our clients and their representatives.

Professional Geriatric Care Managers offer the following services:

Assessment of clients to identify specific areas of need and available financial resources.

Plan of Care is developed to address the needs of the client with recommendations for appropriate services.

Referrals to legal professionals to ensure appropriate measures are in place to protect clients, as well as their client's assets and resources.

Advocacy and guidance in the health care arena, including referrals to medical professionals who specialize in areas of concern.

Implementation of the Plan of Care to confirm that appropriate services are set up and professionally overseen.

Monitoring of ongoing client needs and updating the Plan of Care as necessary to ensure that clients continue to receive the care they need, when they need it.



Additional services offered by many Geriatric Care Managers include:

- Crisis intervention services

- Acting as a liaison to families and alerting families to problems

- Assisting clients with moves to assisted living facilities and nursing homes

- Offering counseling and support through individual therapy, conservatorship or guardianship

- Coordination of medical care and appointments, including transportation to and attendance at physician appointments.

- Set up of weekly medication with monitoring of compliance

- Arranging for medical equipment and home care services

- Regular visits to the client to monitor status and ongoing needs
